I am a regular member of LTVTP-MPC and hereby agree to participate in the Mutual Aid Fund program under the following terms and conditions:
1. ELIGIBILITY AND PAYMENT
- I confirm that I am a regular member of LTVTP-MPC.
- I agree to pay One Thousand Pesos (₱1,000.00) upon joining this program.
- I understand that there is a one-month contestability period before claims can be made.
2. RENEWAL AND FORFEITURE
- I acknowledge that the Mutual Aid Fund will be automatically renewed from the ICPR every distribution, regardless of my month of membership.
- I understand that if no payment is made within the anniversary year and I die, no claims shall be given.
- Claims must be made before the anniversary of payment ends; beyond this, they will be forfeited.
3. CLAIM AMOUNTS
I understand that the claimable amount depends on my share capital and years of membership:
For members with minimum share capital of ₱20,000.00 and above:
- 10 years and above membership: ₱120,000.00
- 5 to 9 years membership: ₱80,000.00
- 4 years and below membership: ₱60,000.00
For members with share capital below ₱20,000.00:
- ₱30,000.00 claimable amount regardless of years of membership.
4. CLAIM REQUIREMENTS
I acknowledge that the following documents are required for claiming:
- Original or Certified True Copy of Death Certificate
- Photocopy of Proof of Beneficiary (Birth Certificate, Marriage Certificate)
- Photocopy with three identical signatures of Coop ID or valid government-issued ID
- Attending Physician Statement (for hospital death or members with loan)
- Barangay Certification (for home death)
- Loan Ledger (for members with loan)
- Claimant’s Statement
- All passbooks of the member must be surrendered
5. CLAIMS PROCEDURE
I understand the claims procedure as outlined in the Mutual Aid Fund policy.
6. MODIFICATION OF TERMS
I acknowledge that LTVTP-MPC reserves the right to modify the terms of this Mutual Aid Fund, and that I will be notified of any changes.
