Qualifications
- Applicant must be residing in Luzon
- Filipino Citizen
- Of legal age, 18 to 65 years old
Basic Requirements
- He/She has satisfactory completed the Pre-Membership Education Seminar
- Accomplish the Cooperative’s Membership Application Form
- Submit four (4) pieces of ID picture in any size. (2×2, 1.5×1.5, or Passport Size)
- Photocopy of Identification. (1 Primary ID, 2 Secondary ID’s)
- Tax Identification Number (TIN)
- Paid Monetary Requirements

How to Apply
Get Started in Just a Few Steps
Attend the PMES (Orientation)
Complete the Pre-Membership Education Seminar (PMES) to understand the cooperative’s services, policies, and responsibilities.
Fill Out the Application Form
Accomplish the official Membership Application Form of the cooperative.
Prepare your Requirements
Submit all required documents.
Pay the Required Capital
Settle the initial share capital and other applicable membership fees based on your chosen membership type.
Membership Approval
Your application will be reviewed for approval. Once approved, you officially become a member of the cooperative.
